Sacramental preparation information can be submitted to the St. Olaf Director of Relgious Education by completing the online information below OR by printing, completing, and sending the form to the parish office along with a copy of your child's baptismal certificate. If your child was baptized at either St. Olaf in DeForest or St. Joseph in East Bristol - no need to send a copy of a certificate as we have your information on file.
